Integrated Assessment Tool and Person-centered Process
COLLAGE, The Art & Science of Healthy Aging advances healthy aging and improves outcomes. COLLAGE was created as a joint venture between Kendal Outreach, LLC (KOLLC) and the Institute for Aging Research/Hebrew SeniorLife, a not-for-profit organization affiliated with Harvard Medical School.
COLLAGE is a customized suite of standardized and systematic resident/client assessments that evaluate health and wellness in areas such as memory, nutrition, balance and mental well-being. Assessment data is gathered from a consortium network consisting of continuing care retirement communities (CCRCs), congregate senior housing sites, both market rate and subsidized apartment dwellings, and senior service agencies. The data is then collected in a national repository and disseminated to consortium members.
COLLAGE reports are provided to the COLLAGE membership on two platforms: 1) individual resident assessment that leads to individualized service plans and 2) organizational performance assessment on individual housing sites or service centers that seek effective interventions, preventive programs and focused services.
